6. Select option and brand of vaccine. “Next” to continue.
7. Answer eligibility questions and complete attestation. “Next” to continue.
8. Provide prescription insurance details
9. Enter zip code “01702”
10. Select “MetroWest Pharmacy – 214 Union Ave, FRAMINGHAM MA 01702”
11. Select date of appointment
12. Select time of appointment
13. Accept terms and conditions
14. Click “Confirm” to book your appointment
15. Print or email vaccine voucher
16. Bring the following to your appointment:
    COVID-19 vaccine card
    Insurance card (or Medicare Part B card if over 65)
    Photo ID
17. Confirmation sent to your email once appointment is scheduled
